If you are applying for a CNA position, you must apply for the state specific CNA/LNA license with the appropriate State licensing organization.

If you intend to continue practicing in Vermont after March 31, 2022, you will need to re-register through OPR’s online services system.  To apply, please follow these step-by-step instructions. 

  1. Register an account. https://sos.vermont.gov/opr/online-services/(do not register if you currently hold or have held a license with the office). Retain Your User ID and Password.
  2. Enter the user ID and password you registered and created. Click the “Log in”
  3. Click “Apply for Facility Staff/Inactive Registration”.
  4. Answer all questions within the application and hit next to move to the next screen. When you get to the end of the application, click “Submit” to complete the process. 
  5. A confirmation email will be sent to the email provided with confirmation the application has been received.  Please allow 3-5 business days for processing and watch your email for updates. 

Finally, should know that there is an alternative to re-registering:  If your out-of-state license has been active in good standing for three or more years, you may qualify to quickly and easily obtain a conventional Vermont license that confers unrestricted practice authority, regardless of work setting.  The process is simple.  Please visit our fast-track endorsement webpage for more information.
